When I went to renew I saw an eye doctor first, so I could get new glasses and pass the DMV test, but the eye doctor said my eyes were easily good enough to pass without glasses, so he gave me a form to submit to DMV. Point is, I thought as I get older my eyes would grow weaker, but in fact I see better now than at any time in the past 50 years.

Rather than asking here, I think you should write to or call the DMV--it's their rule. I always thought that whatever your legal name was, is what goes on your license. If you moved your maiden name to your legal middle name when you got married, that's what would be used.
Not to disappoint you, but couple of my friends were failed apparently for no reason. Even i was there when they gave the test. The only reason the examiner gave was a cross hands while looping in the circle at Bakers Basin circuit. I think, cross hands may cut a few points during a test, but should not be the only reason to fail someone in the test. Sometimes luck just didnt work in ones ...
